So, what's missing? More precisely - what's stopping you from dumping
mailman or ezmlm or whatever?

doc/TODO says

I may try and write this tomorrow.

There's kind of one there already but, as richard pointed out, it could
do with some work - instead of telling people how to do it in the
installation guide it should try and do it in code. There also needs to
be better provisions for installations not using SQLite.

See above.

I'm not quite sure how to do this yet.

Done, although Richard thinks that there could be a cleaner way of doing
it.

Doesn't interest me much but shouldn't be that hard at all.

The one thing I've been thinking of is sending out to users. At the
moment the Send plugin loops though all the users and send a mail out to
each one. The thing is - for us to do things like per-user NoMail and
PGP and ReplyTo or whatever instead of just looping through plugins
before we get to Send we're going to need to do it when we loop through
all the users as well.

The big question - do we codify this as part of the design or leave it
as a plugin implementation detail?
